T1 with Voice and Data

Can you provide a “Big Picture” description of what is used when a company has a T1 for telephone service and Internet (Fractional)?
I am looking for a description of what equipment, technology method (how it works on your side and the Provider’s side) and service agreements may be used from someone who has been actively involved in this service.
