Windows proxy server. Best way to block inappropriate sites?

I need opinions on what the best way is to block inapproriate sites. Despite the presence of URL filters, we have found that users use Web proxies to make an indirect connection to the Internet through an outside proxy server. How can this be avoided? and how can the inappropriate sites be blocked?
