Windows server support knowledge transition plan

Please can you provide a sample project plan (XLS or MPP) for Windows Server Infrastructure support knowledge transition. I am looking for a template that I can use as a reference during the knowledge transition phase for taking over support of my clients windows server infrastructure. I am looking for a technical step by step list of activities to be performed during the knowledge transition phase.
