Duplicate copy prints solid black

I have an HP 4050 and on occasion if I print 2 copies of a document from any given program, it prints the second copy solid black. I have disconnected all of the cables and turned everything off and then reconnected them. It is hooked up to a jetdirect card and on the network. So, this happens with more than just one person. I’m thinking either a driver issue or something with the fonts… can someone help with this issue?
