Modem problems Win 98SE

I’m having problems with my Web connection.
Have win98se, USRobotics 56k fax modem.
Using dial-up networking to a local ISP.
My connection speed is great, but the two connectivity indicator (two little connected boxes on lower RH corner of screen) don’t stay lit.

Thought the problem was my modem so I replace it. I ran a modem test utility and got the following message:
