NTFS Partition Failure

How do I fix the following?
1. D:\ partition of hard drive can’t be read.
2. I can’t even see the properties in Windows Explorer.
3. In Disk Administrator it shows a FAT partition but I know it is NTFS.
4. I ran “autochk” and it showed fixing some file errors and some log errors.
