Slow computer

I have a user that has been having problems with her computer on and off for months. It seems to be running extrememly slow, I have reformatted it, I have cleaned it up, defraged it. I am stumped I do not know why it is still running slow, does anyone have any ideas. Please help if you can. Thanks
