Advanced Search Form

I am looking for an advanced search form template/example/tutorial that pulls from multiple fields in a database (title, author, keyword etc). I am able to do the simple search fine, but am not understanding how to get the form to search formultiple fields in the database. When I hit the search form I get all the results in the database. If it is helpful - I am using SQL 7 and Interdev to develop the form.

Using SQL Server 2000, improved their search capability to accurately cover 100 percent of their catalog, where previously they could only cover 70 to 80 percent of their SKUs. SQL Server's Full Text Search capability enables site visitors to search by partial word and partial part number, enhancing search results by an estimated 100 percent hit rate. SQL Server also provides a better solution for SKU latency, so product additions and price changes can be reflected immediately, rather than the 24 hours required with their previous application. Performance of the search engine is also improved using a search caching technique developed by and Microsoft engineers, which caches search results in SQL Server tables, dynamically created for the user session. This caching provides's users with a 30 percent improvement in ASP requests per second.
