App selection for a novice

I am a startup and expert in my field. I have zero experience with apps however and very little tech experience. I have spent weeks combing through websites and reviews looking for the right app for my business needs. I am no closer to finding one. I am hoping that someone can steer me in the right direction...
I may not have the correct terminology but please bear with me. I require three essential elements;
A reference database
-database with tables containing prefilled fields which can be edited/added to
-used for all projects
A project database
-database containing pre filled table headings i.e. Column and row titles with mostly empty fields
-used for individual projects
-user friendly mobile form which can be used offline for iPad/iPad Pro
-multiple choice/yes/no and long text options
-skip/conditional logic
-seperate 'pages' preferred over long form

All three 'elements' need to relate to each other. For example; a single answer on the form accesses three seperate fields from the reference database and 'pastes' them to predetermined fields on a project database.

Please note. I know a custom app would be best but my budget is small. I am hoping to be able to learn a little coding if required.

Please respond in tech language that my 12 year old might understand as it appears she understands as much as me at this point!

Native application - cost efficient

Please check We have a platform that does native applications in fraction of the time and cost.

What you are describing can easily be done in couple of thousand.
