practices listing roles & responsibilities that
include how a tool is modified, version control,
file maintenance, password control, special
requests, etc.

Use Microsoft Visual Source Safe control for version and configuration management, it has built in password security, and any type of file can be managed using this tool

You'll always find lots of opinions about best practices and that too about "written best procatices".

For the above specific need, you might like to try the to know about Rational Unified Process which is built around UML. While it is a generictool, I found that it is also descriptive and exhaustive enough to cover most scenarios.
