Creating Forms in Outlook 2000

I am trying to create a Work Order form in Outlook 2000. It is my desire that when any staff memebers have difficulties with their systems they would have the ability to access this form within Outlook, fill in the necessary info and the form would be automatically sent to the IT dept. I've searched thru the help topics and they all tell me to Publish this form to the Organizational Form Library. Can't find this Library. And the Outlook Library doesn't seem to accept it. Does anyone have any ideas??

I use many different standard email documents to send and confirm projects with clients. To do this I create a regular word document with the information and layout I require, click on the email icon in the menu, type in the required "TO" "CC" and "BCC" addresses. Then, name the file and save it with an .OFT extension. I save the file to the desktop for easy access. Each time you click on the icon the template is opened as it was created or last saved.
