Databases that failed

I'm looking for examples of database systems that failed. I note the the FBI has provided a new example: "The bureau is so convinced that the software, known as Virtual Case File, will not work as planned that it has taken steps to begin soliciting proposals from outside contractors for new software, officials said. So far the
overhaul has cost $581 million".

Does anyone have other examples of expensive failures? The Canadian Gun Registry? The two Washington State Ferry System databases? Others?

Ontario Land Registry Database

From the provincial auditor's 2000 report.

"In 1991, the POLARIS project had an original cost estimate of $275 million and an anticipated 1999 completion date. In April 1999, Teranet provided the Ministry with an estimate of over $700 million to complete the project and a projected completion date of 2010."

There you go, I'd have to say that one was off the mark.
