End User Survey

I would like to gather some feedback from the user community on how they feel the IT department is performing. Without having to reinvent the wheel I was wondering if anyone had a survey they used and would care to pass it on.

My approach is to send the user an e-mail with voting buttons to make sure they were happy with the service they received as the ticket is about to be closed. This means that the user has the oppertunity to provide feedback which my department can learn from.

Sorry Vawns at this stage we do not use a very elaborate Help Desk system. I was looking more in the area of a 20 question survey. Thanks
