Forms to Reports?

I am working on a database that automatically generates default dates thru a query into a form. These values are not stored in my table, I was told they could not be due to time being added or subtracted from another date field
in the table. My employer is asking me to generate reports that show a specific description and the value. I do not know how to accomplish this and hope someone can help me.
