Helpdesk Customer Survey Software?

We are in the process of updating our survey tracking software on our helpdesk. We need something that can use FTP from Remedy, send a survey to the user and collect a reply from the end user to generate a survey report. Our software is old and outdated and the company has gone out of business so we can no longer get support. Any suggestions or leads would be appreciated.
