How to create an url to specific document AND bookmark

Try to create an url to open an Adobe Pdf file (version 6.0) and to a specific bookmark.
Already read PDF Open Parameters (tecnical note # 5428) but cannot find the answer.

The url (which is not working is):
http://corporate.site.local/document.pfd##nameddest=hoofdstukD”

Who know how to make this url work or how to convert bookmarks into named destinations automaticly?
