I need a form created

I need a form created. It is a Retail installment contract. It needs have the text fixed and not allow editing but the other fields that are able to be filled in and do autocalculation in the other. It cannot be web based as it would reside on sales reps laptops in the field.

I assume the best file format is Acrobat pdf but I need to find someone who can program this to do all the calculations. Or is there a better file format? Again, the "legaleze" cannot be altered by the sales rep.

take a course or read a good technical book on how to use Adobe Acrobat 8. If you learn how, will be cheaper than paying a consultant each time you need a form. :)

Get PDF Pro, it is cheaper..

It will do similar to Adobe, but is cheaper.
