I am trying to find a doc., spreadsheet doc, or some form that my clients can post or write on problems that accur with their software, PCs, etc..

You don't need a 'doc' or a manually keyed spreadsheet, you need a PROCESS.

You'd be well served (and so would your clients) If you looked into a Help-desk database. "How" your customers put the information into it, should be as flexible as to "How" you (and your Clients company) get the information out of it.

B4 you drop any $$ on expensive solutions, determine who is driving this need, does it have company support at the highest level, and what options you have at your avail. Do you want to build your own INTRANET web front end solution feeding a backend database? Do you want to buy a turnkey helpdesk package? What is your clients size? 1-50, 50-100, 100-?? Makes lots of difference...
