As long as you have configured Outlook /Express for your external mail service, should be automatic and no further configuration necessary if you created packet filters allowing traffic on the ports needed to be open. All inbound/oubound traffic goes though ISA server so when a request for outbound traffic is initiated, ISA server checks the appropriate rules, policy and filters. If allowed fowards the request out. Same for inbound traffic.

Here is some info of my ISA Server. There are default protocol definitions upon installation of ISA Server which i think is ok for the usual operations. There is POP3(outbound),POP3 Server(inbound),SMTP(outbound),SMTP Server(inbound). And i create a protocol rules that allows all mail related protocol such as POP3,SMTP,NNTP,IMAP which settings are defined by protocol definition. As far as i know this is the only thing i will need to allow a traffic to pass through. But then when i setup my outlook and setup the incoming/outgoing mail server it cannot send/receive email. My internet is fine using Web proxy as client.
