mail server continuously downloads mail

My client uses a local dial up account to access e-mail. Whenever he logs on to download his mail using Outlook Express the local ISP mail server starts downloading around 50 e-mails from people and sources he does not know. When he logs into his e-mail server through a web based retrieval site that the ISP told him to use it shows the folder is empty. There are no viruses,I ran scan with updated defs. I emptied all temp and temp int folders along with history, recent, cookies. I checked the add remove programs for any spyware or other programs I recognized as bad. There was nothing I did not recognize as known safe.
Any thoughts???

The problem could point to email virus mblaster or variant if using Win 2K or XP,check the system by using the search files or folders,type mblaster.exe,also see the microsoft knowledge base
for the patch to remove,also check to see if the system has been hacked into.
Use a utility to scan for spyware then use removal tool for this.

Your title says MAIL SERVER... but you talk about the user's outlook client...

1) Create the account in outlook on another pc. Does it still download hundreds of mail?

If it doesn't, scan the heck out of that pc for virii and misc spy programs - and download stinger from mcaffee.
