    microsoft access 2003 forms

    I am a COMPLETE NOVICE with Access, so please keep teminology very simple if you reply. I am trying to create a database with data entry using a form.

    Within this form answers to a range of questions will be “Yes” “No” or “NA”.

    I have produced the form and so far so good.

    What I would like to do though is: if the answer is “No” a further box to appear in the form providing a selection of entries to choose from, to identify who did not carry out the required action. This field is mapped to a column in an existing table.
    Is this possible?

    Secondly when the database is opened, can it automatically open the form at the same time?
