microsoft access 2003 forms

I am a COMPLETE NOVICE with Access, so please keep teminology very simple if you reply. I am trying to create a database with data entry using a form.

Within this form answers to a range of questions will be "Yes" "No" or "NA".

I have produced the form and so far so good.

What I would like to do though is: if the answer is "No" a further box to appear in the form providing a selection of entries to choose from, to identify who did not carry out the required action. This field is mapped to a column in an existing table.
Is this possible?

Secondly when the database is opened, can it automatically open the form at the same time?

Thanks for your reply. The tutorial explains how to create a form, however it does not explain how to have a separate drop down list automatically appear when prompted by an entry of "N" being placed in a field.

I guess from this that it cannot be done.
