I'm not sure which version of Access you are using, but in Access 2K, 2K2, and 2K3 (also previous and versions I believe) there are several database creation wizards that would allow you to create a new database and all tables and fields within the tables that you want to use. The wizard that would appear to be most helpful for you would be the "Service Call Management" template.

To launch the wizard, open access and select File>New... click the database tab in the and double click the desired template. This will launch the wizard, which will prompt you on which tables will be created, and you can select which fields you wish to include on each table. Then you can select style for the forms and reports as well. After the wizard runs, you can then customize the forms/reports as needed for your specific application.

Hope this helps.

To add to the previous post. You can download the template or other templates directly from Microsoft.
