Outlook Contacts Form Not Saving Data

I amended the standard Outlook Contacts form to include 3 additional data fields that are text only.
I used forms admin to make this amended form the default contacts form for the organisation and published it on the Exchange 5.5 server.
The new fields on the form do not save any data.
I have tested the form on a standalone PC and it works fine. What Gives ?
