Outlook Custom Forms

I have 2 Outlook custom forms... one designed 10 years ago, and one I'm designing right now. The original form gets posted into a Exchange 2003 public folder, and once posted, it queries the user if they want to send a message to the user's manager for approval. I cannot get the form I'm developing right now to display that prompt. How do I create / enable "action"? And yes... I have attempted to create a "new" action in the form's ACTION tab, but still don't have any luck.

Using both Outlook 2000 & 2003 to try to get this functionality working.

Any assistance would be appreciated!
