Outlook – Forms

I am attempting make a custom form for use within the calendar. I can create a form based on the IPM.Appointment message class easily enough but I would like to be able to customise the default first page as currently I can only add my custom fields into a seperate page on the form.

If it is easier if anyone can even tell me how to make a completely custom form and be able to specify the IPM.Appointment behaiviours that would be great also.

Basically the appointment default does not allow enough fields although the seperate time and date fields are required and the items are to be used within outlook calendars
