Outlook Forms Weblinks

I have created a contact form in Outlook 98 and would like to add four web address fields (or buttons) that go to different pages on our intranet. Whenever I try to add a different address for each of the web address fields all of them change to that last entry. ie, regardless of how many web address fields I have, they will only go to one address.

Can anyone suggest what I might be doing wrong, or an alternative I should use please?
