Outlook Phantom Contact creating email

Outlook contacts keep showing a phantom contact that has not been used in years. The Phantom is not in the contact file but show up when you look-up contacts when creating an email

When it shows up, what happens if you ask for the Properties of it in the lookup? Are you sure it's not in Contacts? Make sure you don't have any filters in the Contacts Folder view and look again.

If it doesn't show up, try creating a new contact that duplicates the name. If it does create without an error, close Outlook, reopen and it delete it. Then close Outlook, reopen and see if the phantom still shows up.
If you get an error saying it already exists, overwrite it, close Outlook,reopen and delete it. Close Outlook, reopen and see if the phantom still shows up.

Also, make sure that Outlook isn't using a Personal Address File as the lookup. Disconnect any PAB that may be attached.
