System Administration Password Policy

I'd be interested in feedback from people on what policy's they have in place for System Administrators and System passwords.

How do you control which administrator is doing what when multiple administrators have access to a system? If you have limited access to certain systems by certain administrators what happens if they are not contactable and someone else is required to administrate how do you control the password distribution?

A thought is to either split the Administration tasks and have each person responsible for a certain task or have a Central Administrator who delegates the tasks to each of the other Administrators which have all been recorded. You can also give some users Administration priveleges so you can track who is doing what.

one overall in charge

I would have one overall in charge of the network and he have majority of the permission the other would have limited permission to areas that they need. This way you have person have person who is in control to set the permissions on the network and give out control so security is kept in tack.
