Training Survey

I am a software trainer for my agency and I am trying to develop a training survey for our users. I want to get an idea of what types of training classes our users might like to have. We have the MS Office Suite (currently 97, but soon to upgrade to XP).I am stumped as to what questions to ask so I can get the response I am looking for. Has anyone created a survey like this before. If you have would you be willing to share an example or two with me, just to give me a jump start.

Several Types

First type is the open ended survey: What do you want to do with Office 97? Or How would you utilize Word (or Excel), etc.

Second type is conditional: If you had to use Word to create a newspaper column in Word, what would you do first?

Thirdis situational: Bob asks you to create a sales spreadsheet that lists sales over 2 million dollars, what do you need to know to accomplish the task?

Fourth type is multiple choice: Given the margins are set to column width, how do you get to the next column without using the enter key: a, b, c

Basically, what do you want the students to learn from your training. Create a beginning, intermediate and advance survey so you can customize the classes.
