Visual Basic for Word 97

Can you help. I am trying to write a visual basic program for MS Word 97. I have little experience using the visual basic interface. What I am trying to do is to set up an automated fax document which will request data from the user using a pop-up form and when the user enters all data, areas of the document are automatically filled. The problem I’m getting is…
When the user creates a new document from the template it seems as if the visual basic code has been destroyed or muddled. It other words, the data that the user fills in on the custom pop-up form appears on the document in different areas.
When I open the original template and run the visual basic form it works fine. Do you know what I’m doing wrong? or how do I get around it?
I hope I’ve made this sound in english….
