That’s a rather oldfashioned view. In a CMS (like WordPress) there isn’t a clear difference, since more often than not it’s done by one and the same person.
How about the following definition for a transaction oriented app or website: development is about the backend, design is about the frontend.

1) Not sure exactly what you mean by assets, but I would say that developers are responsible for creating(coding) everything on the website
2) They are not, they are creating templates/plans/projects and forward them to developers to write the code under their specifications. However, designers should also understand the basics of coding not to create unreasonable/impossibe solutions
3) It depends, but for the most parts yes. Complex websites that takes thousands of hours to code will be the case, but I can imagine a very simple website from the backend, but with tons of effort put into planning it
4) Both functionality and aesthetics are determined by designers, but developers are the one’s that create the code and make it work -
