3-Com Super Stack II 1695 lost passwords

I have been given the above 3-com switch but not the last passwords used. On the 3-Com site it states that the switch will need to be sent in so that a “special” cable can be attached to the module and the switch reset to the factory defaults. DOES ANYONE KNOW how to reset this switch to the factory defaults? I would be interested in what the cable configuration is also.
