Anyone have a form for wireless network cards (ie Sprint EVDO)

We just ordered a few of Sprints EVDO cards for our users to take when they travel and the higher ups want them to sign something when they take it and bring it back, anyone have anything pre-built by chance?

Its unlimited data so its mostly just for tracking who has it, how long its out, and they’re responsible for its cost if they break or lose it.
