blank Oracle Form?

I have taken an Oracle Form, swapkpm, which works under Banner's guagmnu, and adapted it for a new set of tables to swbpkve. This compiles correctly. I have listed swbpkve in guaobjs. When I invoke swbpkve from guagmnu, I get a blank form with the name swbpkve at the top and the usual Banner icons greyed out. I cannot exit back to guagmnu with the X in the top rIght-hand corner: I have to go back to the web page on which Oracle Forms is floating.

What might I be doing wrong in my new Form? I have not - at least intentionally - change any of the Form events.

There is a bug in Java or Forms which sometimes objects to a combo box in a Form. I replace my combo box with a LOV reference and the Form appeared. I have in my investigation of this problem found evidence of bugs in Banner, Forms and Java, but alas I am not in a position to track them down: I must get on with developing that Form, and other things.
