Check-in / Out template for Outlook?

I have a user who is using an existing Check in / Check out of the office for the staff in her office.
The office has Exchange Server, and Outlook on all the stations, and seems to me a Form could be created and placed in the Public Folders, for folks to Sign in and Sign out with.
Has anyone seen such a template?? I’d be interested in hearing of other software which may do the same thing.. contingeant upon cost.. I’d rather however they do what they can with existing software, and Outlook / Corporate Workgroup / Public Folders.. seems so suited to this.
