Have you checked the terminal setup in your emulation program? This may be what is causing the recovery to fail. CCO says you should be setup: 9600 baud rate,No parity, 8 data bits, 1 stop bit, No flow control.

Then power off, wait, power on and hit "break" within 60 seconds to get the ">" prompt.

For more info, is you have CCO access:

Ha, I know this rake... You should not press computer "break" key. You should check your terminal program what is "Break Sequence" in your specific terminal. For example, in ProcommPlus it's Ctrl-F7, in Telix it was Ctrl-B etc. As far as I know Windows HyperTerm does not have this so it's better to use another terminal programs. Since you break booting process you will be in ROM Monitoring, you should change boot register (see password recovery manual), boot without config, go to privileged mode, load old config and after that set new password and save new configuration.

Good luck.

In case you are connected via your laptop and console cable (using Hyperterminal), you might want to connect an external keyboard to it and hit CTRL-C and the break key while router is booting.

Hope this helps.
