Connect to VNC over wireless network

Hi
i am using a linksys wireless broadband router & i have arround 4-5 systems hooked to it. i have installed VNC server on one of my system in the office (accounts desktop).

now i want to acess my office system (accounts desktop) through VNC when i am at home. but i am unable to identify the ip address of the system (accounts desktop) .

when i click ipconfig it displays the ip address as 192.168.2.146 i.e the address allocated by the router.

when i checked the ip address of the internet connection i found that it is 59.91.225.102.
all the systems hooked to the wireless router network use the internet.

now how do i reach my accounts desktop. using
REALvnc
what are the settings to be done on the host?

i would be really thankful for the advice received..
