Create form filter for customer name and begin date and end date

I have to create a form to filter information on customer name and begin and end dates. Once I click on the form filter, a report must be generated to show the Customer name, begin and end dates and details for the dates resulted. The customer and dates are in one table while the rest of the information to be filtered is in another table.

Can anyone help please….
