Creating a boot disk

I am trying to configure SpinRite to see my external SCSI drive. I am told to create a boot disk with the drivers in it. When I boot XP I have no problem seeing this device. But when I create a bootdisk I can not see it.
The device properties are:
-SCSI Disk Device
-Bus Number 0, Target ID 4, LUIN 0

Ok … and how do I add this to the boot.ini file properly?
Here is what my boot.ini looks like.
