Custom paper size

I have a panason KX-2023 printer that is used to print labels that are 5 x 1-15/16. I can not find how to create a custom paper size. I found knowledge base article-838298 and that the hot fix was in SP2. I installed SP2 and did the reg edit. But I still do not find the custom paper size option. I have gone through server properties and set up a new form with the correct dimensions and can not seem to attach it to the printer.
