Customer Satisfaction Survey

I am trying to put together an online CS survey.
Has anyone used a canned system? If so which ones and how did you like it?

What are some things that I should make sure to ask when I put it together?

If you have a Windows 2003 server, you probably have a copy of Sharepoint on it. You can use Sharepoint to build and run a survey. There are also web based survey sites that you can get access to for a fee.

As for what you should ask, you should always know what you want to find out. Ask your managers what kind of data would be useful.

You certainly want to ask about certain qualitative issues: timeliness, satisfaction with support, speed of resolution etc.

You don't mention what services your customers are accessing - help desk, desktop or whatever. Some associations, like the Help Desk Institute, may have samples.
