Customer service survey - software?

I need to conduct regulary monthly/quaterly surveys of how well my support staff is doing. Is there any software out there that's geared to an IT Manager's needs?

Word document/form

I've been using a word doc/form. I'm able to get the survey out - ask the questions I want, but when my business partners send in the surveys, I have to tally the responses manually... a very lenghty process.

I'm looking for software where I can plug in the quesions, and when I get responses, I can plug in them, and have the software tally the responses, and hopefully get reports from it.

Any ideas?

What About Web Forms?

Why not just use a web based form in an e-mail? People fill it out right in their HTML mail reader, and it will send it back to your server?

Not too fancy, but a simple cgi should be able to take care of that...

I've used a combination of simple web and/or Outlook forms together with an Access(or similar) database backend(using simple queries/reports). Took about a day with very minimal programming experience.

If you have MS Access and Frontpage or Outlook, you can create a pretty simple yet effective automated solution without the need to know CGI, ASP, etc.
