DHCP problems

We have a dhcp server that keeps getting bad_address entries, and sometimes ip conflicts. There is no other dhcp server on the network. Sometimes a machine is given an address but it doesn’t list it in Address Leases, and then the dhcp server assigns it to another machine. Then there is a conflict and a bad_address.
