Documenting Group Policy

Not wanting to reinvent the wheel……

I would like to create a form to document Group Policy links & content. The intent is to have a ‘runbook’ of GP creation to act as a reference for the IT support staff.

Things I would like to see;
* Result of GP
* User or Machine policy
* Policies modified
* User filtering (permissions)
* Highest level OU applied
* Any other OU’s affected (or all downlevel)
* Override status
* Any other special setup

Does anyone have such a template built out that I could copy?
