emergency repair disk creation fails

I am trying to create an emergency repair disk on a windows nt 4.0 workstation (service pack 6a). When it gets to the point of formatting the disk I get the message "Setup cannot format disk" and I cannot create the disk.
I have nothing else opened, I am loggoed on as the local administrator, and this happens with whatever disk I use.
Any Ideas on what is causing this?

Are you able to use the disks on any other floppy drive? At the same time are you able to read any other disks on the existing floppy drive?? Can you format the disk you are currently using on this drive out of the RDISK utility??? The problem seems to be with the drive. Changing the drive may help.

I've seen this before and it had to do with SP 6a's updated drivers.
Did the drive work before the sp upgrade ?
