I am creating an on line form and I want to know how to display the form on line so that it covers the entire screen in a window without a menu bar/icons being visible.

When you create the link for the page, highlight the link or button and select hyperlink, type the hyperlink and click on target frame, select whole page.
Every time the link is clicked the destination page will open just the way you want it.

      Good Luck!

in order to do this you need to turn off the shared borders which contain the menu bars, etc. on the page. do this by clicking on the format menu and selecting shared borders. uncheck all of the borders that you don’t want seen on the page. you might want to turn off the theme for that selected page also.
