Help with Sharepoint Group management

I seek help and advice how to manage the site the best way possible. We are currently storing 20 Gb of documents in quite well organized Document Libraries. Of course there are a lot of subfolders and maybe we should have better used more super folders to be able to make better use of columns and views.

The demand is increasing to give persons access directly to only one folder or even just one document! These persons are not supposed to be able to read the rest of the information and docs that were are storing.

I have so far created own groups and I??m not using the default groups, Visitors, Members, Owners because of the reason that I wanted to be able to give people more accurate permission levels.

The problem I am facing, with increasing amount of people, is how to administer this now! Should I create groups for every access and permission even if it only contains one person?

We have approx. 200 users to our team site at the moment.
