Hotfix to IIS 4.0 - setup.log not found?

We're trying to apply the prmcan4i hotfix to our IIS 4.0 server. Whenever I try to run hotfix.exe I get the following message:
"Hotfix setup could not find the setup.log file in your repair directoy".

I've looked on Microsoft's support db and found Q175960 but the fix recommended there isn't working.

The setup.log file is in fact located in the C:\WINNT\Repair directory. Where does the hotfix expect to find the setup.log file?
