How do I make a bootable USB drive?

I want to make a universal boot USB drive, as in one that will boot any and all (reasonable) PCs. NOT an installation drive, but a portable, (mostly) universal Primary Drive. I've attempted this several times and it always works great at the first boot but, after that, it will only boot the machine on which it was originally used. I don't care if it's Win or Linux, PE or not, as long as I can read/write to the other drives on the PC.
