Inherited NTFS Permissions Broken

We have found that some folders which had been inheriting via AD groups have lost all but the list folder contents.

We have three groups,
nnnnnn_FS (List folder Contents)
nnnnnn_RO (Read Only)
and nnnnn_RW (Everything but full control)
(where nnnnnn is the name of the folder path).

We assign all three groups to a top level folder and add the users to the relevant groups. When you look at the persmissions further down the tree you should see all three groups and the assigned permissions above.

However, we have noticed that after a few weeks somehow the inhetited permissions have reverted to List Folder Contents on a good portion on the sub folders. Unable to change this as they are still being inherited from above all we could do was Replace Permissions Entries On All Child Objects.

The question is, how is this possible and is it a feature of Windows 2k3 server (sp1)that anyone else is familiar with???
